Please note: this form is for requesting a refund for Jumpers set 4 only - which was cancelled due to adverse weather.

Please provide: reason for scratching (along with providing a vet or medical certificate), your name, the dogs name and agility number, your entry reference number, classes entered, and your bank account details.
